Indeed JHPIEGO has done a great job in Indonesia and the results show in the gradual reduction of Maternal Death. However Indonesia has one of the fastest growing HIV epidemics in Asia and recent developments indicate a feminization of the Indonesian epidemic. Until a few years ago most HIV cases had intravenous drug use as its origin. Data on HIV among women in Indonesia are limited but the proportion of registered female HIV cases at national level shows a gradual increase: from 13% in 2001, to 21% in 2007, and currently 33% In June 2010 24% of the HIV care patients were female and 22% was in need for antiviral treatment. Most women infected with HIV are intimate partners of drug users and of men buying sex. The first official health staff most of these HIV women meet is the midwife: either for family planning reasons or to check on their pregnancy. In order to improve early HIV detection, testing and treatment and to stem the HIV epidemic in Indonesia there is an urgent need to improve the HIV knowledge and skills of the midwives. A big opportunity for JHPIEGO to build on their strong professional network and to become a major player in prevention care, support and treatment in Indonesia. Merry Christmas and a Happy New Year.
TRANSFORMING CLINICAL TRAINERS
Congrats Muia for the good work to improve clinical training.

More often than not clinical training should cause behavior change aiming at positively influencing people's attitude to improve practices.

Healthcare professional who have served for a number of years always develop their own ways of doing procedures. Protocols are deviated, shortcuts become a norm and this compromises quality of care/service delivery.
Even when they come for trainings their views are skewed. These are some challenges.

However reasons will be given eg staff shortage, financial constraints, inadequate trainings etc but with willingness to learn, some change is expected.

At the end, the effectiveness of training can as well be evaluated by the extent of transfer of learning that has taken place from classroom to actual practice and a positive change in behavior.

Thanks,

Clinical trainer,

Helping babies breath is an innitiative that MUST be rolled out in a hurry. All skilled health care providers in the delivery room MUST know how to resuscitate the newborn especially considering that most facilities may not have the presence of a Paediatrician and help can come a minute too late! This will assist in reducing the perinatal losses and susequent long term morbidity experienced from what happened to the babies critically in the first five minutes and the subsequent 24 hours.
